Requests to Fill
Listed, in no particular order, are the kinds of paperwork doctors deal with on a daily basis:
Eligibility for employment insurance forms
Sending out appointments for the RHA – a patient is booked by the RHA for something and the appointment is forwarded to the Family Doctor to send to the patient …don’t ask me why
Rewriting “Expired Requisitions” – returned because they have “expired” - who knew Lab Reqs had a best before date?
Rewriting Requisitions for Patients Who Didn’t Show for an Appointment – that’s right, if you don’t show up for an appointment, the Family Doctor has to rewrite the requisition for whatever you missed
Looking up test parameters for the DI department – you need a test done, the Diagnostic imaging department makes the Family Doctor look up the blood tests they need to perform the test … for them.
Requests for Specialist Consultation
Some Specialties/Specialists have their own particular forms that must be used
Some want a referral every time they are asked to see a patient
Special Authorization Forms – in order to discourage some tests from being ordered, in addition to the ordering paperwork, you have to fill out additional paperwork detailing why the patient needs the thing done
Lawyers Letters – do you see all those ads on TV from personal injury lawyers? Who do you think does the paperwork for this law suites?
Insurance Forms and Letters
Sick Notes
Off Work Notes
Return to work notes
Functional Assessment Forms
Workplace accommodations forms
Whatever Workers Compensation is now called where you live Forms – they have their own forms that have to be filled in.
Social Sevices Letters – if the patent is receiving income supplementation, all manner of letters are required for a dizzying amount of gear. My favourites so far, a letter stating the patient needs a new:
Bed
Mattress
Toilet Seat
A letter stating proof of address for a patient – patients are looking for confirmation for lawyer of where kids lived between certain dates during separation
Disability Tax Credit – recommended by many accountants and tax preparation types, this sixteen (16) page form is designed not to grant a disability tax credit. Go have a look at it yourself.
Prescription renewals
Canada Pension Forms
Driver’s Medicals
